Police Record Checks


Every year, all BMHA Team Officials must obtain a "POLICE RECORDS CHECK for persons seeking VOLUNTEER roles primarily to serve the vulnerable sector of the population". This applies to the following positions:

  • coaches
  • assistant coaches
  • trainers
  • team managers

Notes:

  • Police Records Checks obtained for other volunteer activities (i.e. coaching Soccer, leading Scouts, etc.) are NOT valid.
  • ALL BMHA Team Officials must obtain a police check, regardless of your occupation.

Procedure:

  • Obtain the form "POLICE RECORDS CHECK FOR SERVICE WITH THE VULNERABLE SECTOR"
  • Fill out the form with agency defined as Blackburn Minor Hockey Association
  • Show your Convenor or the Vice-President, Administration your original two pieces of valid identification and have this BMHA official sign as a witness in the "Verified by" box on the form
  • Obtain a BMHA association letter (from your coach, convenor, or the Vice-President, Administration) and fill in your name and Team Official position on this letter
  • Submit the form, the letter, and a clear photocopy of two (2) pieces of valid identification to the Orleans branch or downtown branch of the Ottawa Police Services (this may be done as a team or individually). This must be submitted to the Ottawa Police Services by mid-October
  • Alternatively, if you do not have your identification witnessed by a BMHA official on the form, then you must appear in person with the documentation (form and letter) and originals of the required identification to the Orleans branch or downtown branch of the Ottawa Police Services
  • The original document certified by the Ottawa Police Services with a stamp will be returned by mail to you. You must forward this stamped certification of the Police Records Check to the Vice-President, Administration. It is advisable to make a copy and provide it to your manager as part of your team records before submitting the original to the BMHA
